International Launch Services (ILS) has established its independent Failure Review Oversight Board (FROB) that will review the results of the Russian State Commission’s investigation into the Proton launch vehicle failure on 6 September.

On 17 September at 0605 UTC, the Metop-A payload was autonomously switched-off. The satellite is in a safe and stable state, and the recovery has already started.

Shortly after its recent flyby of Saturn’s moon Iapetus, the Cassini probe temporarily went into safe mode. The spacecraft is operating normally again and its instruments are expected to return to normal operations in a few days.

Kazakhstan has accused Russia of not doing enough to ensure the safety of its space launches a day after a Proton M rocket crashed in the Central Asian state. Meanwhile, satellite operator JSAT has ordered a replacement for JCSAT-11, which was destroyed in the accident.
