New GSLV upper stage fails on inaugural flight

Rocket: GSLV-D3; Payload: GSAT-4; Date: 15 April 2010, 1057 UTC; Launch Site: Satish Dhawan Space Centre, India. The first flight of the GSLV-Mk II rocket ended prematurely owing to a third stage failure. The satellite was lost.
