Several factors contributed to Proton/Block DM launch failure

The three GLONASS satellites launched aboard a Proton M/Block DM-03 on 5 December were lost owing to a combination of systemic and human factors, President and Chief Designer of the Rocket-Space Corporation (RSC) Energia Vitaly Lopota was quoted as saying.
